Most respectfully I beg to state that I am a student of Class - VI, Section-A of your school. I could not attend school for a week from the 5th to the 11th instant of this month as I had been suffering from fever for those days. I was under treatment of a registered medical practitioner. A medical cum fitness certificate is attached herewith.
I, therefore request you to grant me the leave of absence for the period and oblige.

অসুস্থতার জন্য অনুপুস্থিতির ছুটি চেয়ে দরখাস্ত ।
শ্রদ্ধেয় মহাশয়,
যথাযত সম্মানের সঙ্গে জানাইতেছি যে আমি আপনার স্কুলের ষষ্ঠ শ্রেণির বিভাগ - A -এর একজন ছাত্র । আমি এইমাসের ৫ থেকে ১১ তারিখ পর্যন্ত এক সপ্তাহ স্কুলে উপস্থিত হইতে পারি নাই । কারণ ঐ কয়েকদিন আমি জ্বরে ভুগিতেছিলাম । আমি ওই সময় একজন ডাক্তারের নিকট চিকিৎসাধীন ছিলাম । আমার এই আবেদনপত্রের সঙ্গে ডাক্তারের একটি শংসাপত্র পেশ করিলাম ।
আপনার নিকট আমার বিশেষ অনুরোধ,ওই সময়ের জন্য অসুস্থতাবশত ছুটি মঞ্জুর করিয়া আমাকে বাধিত করিবেন ।
